ABSTRAK
Pendahuluan: Celah bibir dan langit-langit non sindromik merupakan
kelainan kongenital yang sering ditemukan, etiologinya multifaktorial antara
faktor genetikdan lingkungan. Jalur metabolism asam folat dan homosistein telah
diketahui berperan dalam pathogenesis CBLNS, dan gen yg terlibat, antara lain
MTR, diduga meningkatkan kerentanan terhadap insidensi CBL, tetapi sampai saat
ini belum ada penelitian terkait asam folat dan homosistein berfokus pada gen-gen
tersebut pada populasi anak dengan CBLNS di Indonesia Tujuan: Penelitian ini
bertujuan untuk mengetahui epidemiologi dan analisis molekuler faktor genetik
serta pola polimorfisme gen pada ibu dengan anak penderita CBL. Tujuan jangka
panjang penelitian ini diharapkan dapat dijadikan strategi dalam pengembangan
langkah – langkah pencegahan kejadian CBL di indonesia Metode: Pemeriksaan
homosistein digunakan metode ELISA. Deteksi polimorfisme gen MTR
menggunakan metode PCR-Sanger Sequencing. Ukuran sampel ditentukan
berdasarkan tabel kontingensi, dan pada setiap sel jumlah minimalnya adalah 4.
Variabel terikat yang diperiksa ada 2 dan variable bebas ada 5, sehingga didapat
tabel kontingensi 2 x 4 , oleh karena itu sampel minimal adalah 8 x 5 = 40 sampel.
Hasil: Sebanyak 40 sampel kasus didapatkan genotipe TT (normal) sebanyak
13 genotipe (32,5%), serta genotipe TA dan AA yang merupakan sampel
polimorfisme pada kasus sebanyak 27 sampel (67,5%). Sampel kontrol berjumlah
43, frekuensi genotipeTT (normal) sebanyak 20 genotipe (46,5%), serta genotipe
TA dan AA yang merupakan sampel polimorfisme pada kontrol sebanyak 23
sampel (54,5%). Nilai rata-rata kadar homosistein pada ibu dengan dengan anak
oenderita CBLNS adalah sebesar 911,3 sedangkan pada anak normal sebesar
827,3. Nilai ini mengindikasina bahwa kadar homosistein pada ibu dengan anak
penderita CBLNS lebih tinggi dibandingkan pada ibu dengan anak normal. Dua
variabel dikatakan memiliki korelasi yang sangat tinggi jika memiliki nilai
korelasi lebih dari 0,9. Berdasarkan hasil perhitungan korelasi dapat dilihat bahwa
nilai koefisien korelasi adalah sebesar 0,999. Simpulan: Kadar homosistein
serum darah pada ibu dengan anak penderita celah bibir dan langit – langit lebih
tinggi dibandingkan dengan ibu dengan anak normal. Polimorfisme gen MTR
pada subjek kasus dan kontrol tidak ada perbedaan yang signifikan, sehingga
polimorfisme gen MTR bukanlah salah satu faktor terjadinya kasus celah bibir
dan langit – langit. Korelasi antara kadar homosistein dengan polimorfisme
rs955516 gen MTR dengan kejadian ibu dengan anak penderita celah bibir dan
langit – langit sangat berpengaruh, dan memiliki korelasi yang kuat.

Correlation Between rs955516 (MTR Gene) polymorphism and blood serum
homocysteine levels in mothers with non – syndromic cleft lip and palate child
ABSTRACT
Introduction: Non-syndromic cleft lip and palate is a congenital disorder that is
often found, the etiology is multifactorial between genetic and environmental
factors. The metabolic pathways of folic acid and homocysteine are known to play
a role in the pathogenesis of CBLNS, and the genes involved, including MTR, are
thought to increase the susceptibility to the incidence of CBL, but until now there
have been no studies related to folic acid and homocysteine focusing on these
genes in the pediatric population. with CBLNS in Indonesia. Objectives: This
study aims to determine the epidemiology and molecular analysis of genetic
factors and patterns of gene polymorphism in mothers of children with CBL. The
long-term goal of this research is expected to be used as a strategy in developing
measures to prevent CBL events in Indonesia. Methods Homocysteine
examination used the ELISA method. Detection of MTR gene polymorphisms
using the PCR-Sanger Sequencing method. The sample size is determined based
on the contingency table, and in each cell the minimum number is 4. There are 2
dependent variables examined and 5 independent variables, so that 2 x 4
contingency tables are obtained, therefore the minimum sample is 8 x 5 = 40
samples. Results A total of 40 case samples obtained TT (normal) genotypes of
13 genotypes (32.5%), and TA and AA genotypes which were polymorphism
samples in 27 cases (67.5%). There were 43 control samples, the frequency of the
TT genotype (normal) was 20 genotypes (46.5%), and the TA and AA genotypes
which were polymorphism samples in the control were 23 samples (54.5%). The
average value of homocysteine levels in mothers with children with CBLNS is
911.3 while in normal children it is 827.3. This value indicates that the
homocysteine level in mothers with children with CBLNS is higher than in
mothers with normal children. Two variables are said to have a very high
correlation if they have a correlation value of more than 0.9. Based on the results
of the correlation calculation, it can be seen that the correlation coefficient value
is 0.999. Conclusion: Blood serum homocysteine levels in mothers with children
with cleft lip and palate are higher than mothers with normal children. There was
no significant difference in the MTR gene polymorphism in case and control
subjects, so that the MTR gene polymorphism was not a factor in the occurrence
of cleft lip and palate cases. The correlation between homocysteine levels and the
rs955516 polymorphism of the MTR gene with the incidence of mothers with
children with cleft lip and palate is very influential, and has a strong correlation
