  • Introduction: Bad splits is one of the most common intraoperative complications during bilateral
    sagittal split osteotomy (BSSO) procedure when treating patients with mandibular deformities.
    Adequate management of bad split complication will prevent further post-surgery complications.
    The purpose of this study was to assess the treatment option for bad split complications during a
    BSSO procedure. Methods: The current literature review was performed by assessing articles about
    the management of bad split complication during a BSSO procedure based on the Preferred
    Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. Articles were
    obtained through a manual searching from several search engines namely Pubmed, ScienceDirect,
    Ebscohost, Scopus, Medline, Embase, and Web of Science and also through snowball technique. All
    authors were independently involved in the establishment of inclusion criteria, data extraction
    process, and determination of the risk of bias. The screening process was conducted by applying the
    inclusion and exclusion criteria. Results: Seven articles that consisted of five case series and two
    cross sectional studies were included in this systematic review. During data extraction process, it
    was revealed that the usage of an osteosynthetic plate is the most used procedure in managing bad
    split complication during a BSSO procedure. Conclusion: The treatment of choice for bad split
    complications is the osteosynthetic plate and miniplate application with bicortical screw or
    monocortical screw.
