  • Introduction: Dental procedures in the lower mandible has a risk of damaging the
    inferior alveolar nerve (IAN) and blood vessels within the mandibular canal. This study
    is conducted to observe the anatomical position of the mandibular canal in respect to
    tooth apices in the buccolingual dimension and its contact relation to the roots.
    Methods: This cross-sectional study include previously taken computed tomography
    (CT) scans of 75 patients (43 men, 32 women) selected from an available database in
    Hasan Sadikin General Hospital, yielding a total of 516 roots from 301 teeth of the
    second premolar to the third molar. The buccolingual position and contact relation of
    the canal to the root apices were determined. The results were then observed based on
    sex and age. Results: The mandibular canal were located apically to the apices 58.7%
    of the time. Out of 516 roots, 103 were in direct contact with the canal. The highest
    frequency of a contact relationship between the canal and the apices was in the distal
    root (43.4%) and mesial root (37.3%) of the third molar. The lowest frequency was
    reported in the mesial and distal root of the first molar (1.5%). Conclusion: Direct
    contact was found in the apices of premolars and molars through CT images. Dental
    practitioners should consider the relationship between the two structures during dental
    procedures to avoid iatrogenic damage to the nerve.
